NOTE: Make sure that you inform all participants that the information they share is confidential and will be kept confidential. No names or identifying information should be included in the final report!!
Parent Survey
1. Describe your child.
2. Describe services your child receives.
3. Do you believe that your child receives all the services he or she needs?
(if no, what services would you like your child to receive?)
4. How are you included in determining the services your child receives?
5. How often do you communicate with service providers regarding your child? (Is it often enough? How do you communicate?)
6. What advise do you have for a beginning teacher of young children?
Special Education Preschool Teacher
1. Describe what you do. (Some of this may be answered in later questions)
2. Describe the children in your classroom (no names).
3. Describe your classroom (setting, schedule, activities, etc.)
4. Describe your interactions with the families of children you serve. (How, type, frequency, etc.)
5. How are IEP meetings implemented? Describe a typical sequence for a child’s IEP meeting.
6. How are you involved in transition planning and implementation for 3-year-olds?
7. What is reinforcing about your position?
8. Given the opportunity, what would you change about your position?
Preschool Teacher
1. Describe your classroom (setting, schedule, activities, etc.)
2. Do you serve children with disabilities? If so, which disabilities? What supports are available for the children?
3. Have you referred children for special education testing? If so, what prompted the referral?
4. Have you attended IEP meetings? If so, describe your role in the meeting.
5. Describe your interactions with the families of children you serve. (How, type, frequency, etc.)
6. What is reinforcing about your position?
7. Given the opportunity, what would you change about your position?
